This draft describes the Canadian Hospital Education Partnership (“CHEP”) and the personal information collected through chepcanada.com. It is written with the Personal Information Protection and Electronic Documents Act (PIPEDA) in mind. It is not a legal opinion and it does not state that a privacy impact assessment has been completed.

Partnership applications collect organization details, workforce estimates, education arrangements, and the name, position, email, and telephone number of a primary contact, plus the narrative answers on the form.

The contact form collects a name, organization, email, and message.

Server logs may include a hashed form of the network address used to submit a form, kept for abuse prevention. CHEP does not ask applicants for a social insurance number, a health number, or patient information. Do not include patient information in a form.

We use application information to review eligibility, plan capacity, and contact the organization if the person consented. We use contact messages to reply. We use administrator accounts to operate the site. We do not sell personal information.

Staff may add internal notes to an application. Those notes are for CHEP review. They are not published on the public site and they are not included in the public partner list.

A draft process for access, correction, and questions will be confirmed with counsel. Until a privacy office is named, write to partnerships@chepcanada.com and mark the message “Privacy”.
